About the Role:
This is a part-time, Fixed Term position (until 30th June 2027) at 0.5FTE, 38 hours per fortnight within the Department of Adolescent Medicine, Young People’s Health Service (YPHS). YPHS is a nurse-led primary health care service that aims to advance the health of young people who are who are at risk of, or are experiencing homelessness. This position is offsite to RCH and is located in the Melbourne CBD.
You will be responsible for improving access to government funded vaccines for vulnerable young people attending flexible learning centres, short term refuge accommodation and the YPHS Nurse-led clinic. In addition to providing direct clinic care you will also provide secondary consultation and support to Nurse Immunisers in the community.
Internally, this position will be referred to and known as: Community Health Nurse - Immunisation Outreach Project.
Classification for this position is governed by Award 22 - Registered Nurses (Community Health Nurse), ZJ1 at $115,289.20 per annum, pro rata for part-time, plus Superannuation.
What you’ll achieve:
- Support the delivery of individualised care based on assessed needs and evidence-based practices
- Advocate for the needs and preferences of young people, ensuring their voices are heard in care decisions
- Foster effective communication within the healthcare team, promoting a collaborative and cohesive working environment to ensure coordinated and effective patient care
- Provide education to upskill other staff, supporting their professional development
Your skills and experience:
Essential:
- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) administered through the Australian Health Practitioner Regulation Agency (AHPRA)
- Demonstrated commitment to ongoing personal and professional development as evidenced in a Continuing Professional Development (CPD) record/Professional Practice Portfolio
- Accreditation as a Nurse Immuniser
- Experience in Adolescent, young adult &/or community health nursing
Desirable:
- Experience in sexual and reproductive health nursing

About the Department:
The Department of Adolescent Medicine provides care and management to adolescents and their families via a range of specialist multidisciplinary services including the Gender Service, Eating Disorders Service, Chronic Illness Peer Support (ChIPS), Young People’s Health Services and inpatient and outpatient medical service.
